  • Frequent visits to the Emergency Room and/or hospitalizations
  • The resident declining medical treatments
  • Significant weight loss due to inability to eat or lack of desire to eat

  • Frequent or increased falls
  • Decreased focus, increased sleeping, withdrawal, or confusion
  • Focus shifting from aggressive treatment to quality of life

We work in conjunction with local hospice programs and the residents’ medical team to provide individualized care.

  • Our experienced care team provides physical and emotional comfort to seniors when hospice care is beneficial.
  • One of the main objectives of hospice care is to allow residents to live with dignity and be free of pain.
  • During this time, residents also have the option to take part in the other amenities that Country Woods offers, including visits from clergy and social services if desired and/or needed.
